Clinical Interpretation of Periodontal Signs and Symptoms and Their Prognostic Value Before, During, and After Orthodontic Tooth Movement

The interaction between periodontology and orthodontics is one of the most challenging aspects of treatment planning in patients with a history of periodontitis. The success of orthodontic therapy depends not only on correcting malocclusion but also on the clinician’s ability to accurately interpret the clinical and radiographic signs that reflect periodontal stability or disease progression. This lecture will discuss the prognostic value of the main periodontal parameters before, during, and after orthodontic tooth movement, with particular emphasis on patients affected by Stage III and Stage IV periodontitis. The clinical significance of probing pocket depth, bleeding on probing, tooth mobility, gingival recession, clinical attachment loss, and periodontal phenotype will be examined to identify the conditions that allow orthodontic treatment to be safely initiated and those that require additional periodontal therapy. Special attention will be devoted to the interpretation of residual periodontal pockets, bleeding on probing, and tooth mobility following cause-related periodontal therapy. Clinical decision-making criteria will be discussed to determine whether continued non-surgical therapy, periodontal surgery before orthodontic treatment, or the initiation of orthodontic tooth movement under conditions of periodontal stability is the most appropriate therapeutic approach. The lecture will also address the clinical significance of the persistence or recurrence of these signs during and after orthodontic treatment, highlighting when they represent physiological tissue adaptation and when they require further diagnostic evaluation and additional periodontal intervention. A further focus will be placed on the indications and frequency of radiographic monitoring during orthodontic treatment, following the principles of appropriate prescribing, as well as on the optimal timing of periodontal surgery in relation to orthodontic tooth movement. By integrating the most recent scientific evidence with clinical case presentations, this lecture aims to provide practical, evidence-based decision-making criteria to optimize the interdisciplinary management of periodontal and orthodontic patients, ultimately improving the long-term prognosis of natural teeth and the stability of treatment outcomes.
