Abstract
The Stubborn Impacted Tooth: to Move or to Sacrifice?

Impacted teeth remain among the most demanding challenges in orthodontics, requiring clinicians to balance biological limitations, patient expectations, treatment timing and biomechanical feasibility. Successful outcomes rely not only on technical expertise, but also on sound clinical judgment and evidence-based decisions. This lecture will offer a practical and educational perspective on the diagnostic and therapeutic dilemmas associated with challenging impactions. Diverse clinical scenarios involving canines, incisors and molars will be presented to illustrate the rationale behind therapeutic choices and the complexities commonly encountered in daily practice. Particular emphasis will be placed on the key factors guiding decision making, including patient age, timing of intervention, severity and location of impaction, root development, periodontal and esthetic considerations, and biomechanical limitations. Through carefully selected cases including both successes and complications, the lecture will address critical questions frequently faced by clinicians: When should an impacted tooth be orthodontically recovered? When should treatment plans be modified? And when do biological risks outweigh potential benefits? Scientific evidence will be integrated with long-term clinical experience to demonstrate strategies for minimizing iatrogenic effects, improving predictability and managing complications.
