Orthodontics and DTM: What the Literature Says

The potential link between orthodontic treatment and temporomandibular joint (TMJ) dysfunction has been a subject of debate since the landmark 1987 U.S. court case *Brimm v. Malloy*. In that case, a patient treated with premolar extractions, fixed appliances, and extraoral traction developed a dysfunction and successfully sued the treating orthodontist. Subsequent research studies refuted the theory that orthodontic treatment was a contributing factor. The presentation will review these studies, discuss issues regarding the etiology of TMJ disorders, and highlight the importance of distinguishing between muscular and articular problems—with a particular focus on disc displacements. Scientific articles analyzing the prevalence of disc displacements in young patients—and the challenges involved in identifying them—will be discussed. The presentation will also address the importance of correcting crossbites to prevent displacement, as well as the relationship between deep bites and disc displacement. Practical clinical tips for detecting potential displacements without the need to order an MRI scan will be suggested.
