Orthodontist–Periodontist Integration: Treatment Objectives and Clinical Coordination

Approximately 50% of the adult population is affected by periodontal disease, with pathologic tooth migration occurring in 30-55% of cases, necessitating an interdisciplinary approach. Healed periodontal patients present a unique biological challenge: orthodontic tooth movement (OTM) acts as a "sterile" pseudo-inflammatory process. If plaque is present, OTM can trigger active inflammation, leading to attachment and alveolar bone loss. The minimum requirements to commence tooth movement include confirmed periodontal stability: no sites with PD ≥ 4mm and BOP+, and both Full Mouth Plaque (FMPS) and Bleeding (FMBS) scores below 10%. In such cases, it is often appropriate to prioritize functional stability and long-term tissue health over ideal aesthetic goals. Clinical planning may involve intrusion, extrusion, and all dental movements; literature supports these movements in reduced but healthy periodontium, provided tissue healing times are respected. Orthodontists must evaluate various therapeutic options, ranging from continuous archwire approach and clear aligners to segmented techniques supported by Temporary Anchorage Devices (TADs). TADs are specifically indicated to avoid unnecessary biological stress or "round-tripping" on compromised anchor units. To guide these complex decisions, we present the OPRA (Orthodontic-Periodontal Risk Assessment) chart, a procedural algorithm evaluating six vectors: anchorage unit preservation, device cleansability, treatment time, force system accuracy, alveolar bone housing/gingival phenotype, and individual susceptibility (Stage and Grade). In conclusion, successful treatment is only achievable through a coordinated team approach. The OPRA chart serves as a vital tool to quantify risk and define the frequency of maintenance appointments via the "Stop & Go" protocol, ensuring periodontal stability is monitored throughout the orthodontic phase.
